Reduced Fare Metrocard Services

Reduced Fare Metrocard Services are coming back to Brooklyn Borough Hall.
Wednesday, March 25, 2015; Wednesday, April 22, 2015; and Wednesday, May 27, 2015
1 PM – 3 PM
AN MTA NYC TRANSIT REPRESENTATIVE WILL BE PRESENT TO ASSIST WITH THE FOLLOWING SERVICES:
•Initiate the application process for the Reduced-Fare MetroCard.
•Seniors (65+) and People with Qualifying Disabilities can receive a temporary Reduced-Fare MetroCard the same day.
SENIOR CITIZEN APPLICANTS (65+)Must bring (original or copy) of proof of age:
•Medicare Card, Drivers/Non Drivers license,Birth Certificate, Passport
PEOPLE WITH QUALIFYING DISABILITIES
Medicare Recipients must bring:
•Copy of Medicare Card and a Government-issued Photo ID (Drivers/Non Drivers license).
Non-Medicare Recipients must:
•Obtain a Reduced-Fare Disability Application; to be completed by a certified physician and bring a Government-issued Photo ID
(Drivers/Non Drivers license).
•SSI (Supplemental Security Income) recipients, due to Serious Mental Illness must provide their Social Security Number for
verification and bring a Government-issued Photo ID (Drivers/Non Drivers license).
Walk-ins are accepted on a first come, first served basis.
